2008-09 Program Year
The 2008-09 data collection tools/instruments and reporting forms on this page are newly developed for local Community HealthCorps programs. Much was learned from the pilot test, and changes were incorporated accordingly. If you do not see a data tool that was part of the pilot test, then further development is needed before final release. Previously used logs such as for attendance tracking are still allowable.
The main goal is for HealthCorps members to be able to track and report data from encounters to outcomes more easily. Therefore, if you need a tool that is not listed on this page, then contact Savolia Spottswood at sspottswood@nachc.com to explore the potential for creating additional tools.

Additional Guidance for using the Member Summary Reporting Tool
For Large Scale Activities to Increase Access: Members do not have to record "Service Recipient ID's" for the Increasing Access measure. Each row of reporting can be daily subtotals if a local tool/database allows for tracking by individual (i.e. if 20 people were screened for eligibility determination on 09/30/08, then that row would show '20' in the corresponding field.)

Volunteer Satisfaction Survey

This survey assesses the experiences of non-AmeriCorps volunteers that serve in local communities as related to HealthCorps activity. Volunteer feedback is critical - an opportunity to improve programs, events, as well as volunteer recruitment and retention. |

Group Progress Tracker: Case Management & Navigation Assistance

A tool for HealthCorps members to document the assistance they provide to patients and other service recipients for case management and navigation-related activity. Progress can be tracked for up to 5 service recipients per form. |

Introduction to Performance Measurement for Community HealthCorps Programs

A presentation tool for program coordinators that assists with introducing performance measurement to Community HealthCorps members. Use this with handout such as the Glossary of Terms from the 2007-08 Performance Measurement Guidance to provide members with an enhanced training experience. |
