NACHC has teamed with The Dartmouth Institute (TDI), a leading organization in health care research and clinical practice transformation, and Lovett Collins Associates LLC (LCA), a highly experienced team of health consultants, to ready Community Health Centers to become accountable care organizations, or other innovative, integrated models.
CHCs can participate in one or more of the following:
- Conduct individual CHC readiness assessments
- Conduct regional (up to 12 CHCs) readiness assessment
- Join Brookings-Dartmouth Learning Network (BDLN)
- Join a basic (4 month) implementation group
- Develop an ACO road map as part of a regional collaboration
CHC participation can lead to more comprehensive project assistance, resulting in design and implementation of specific local community initiatives.
