This session will explore the trends in health care cost and quality in the US and how our costs of care continue to rise without a return of higher quality care. The concepts underlying accountable care will also be explored. The first webinar in the series explores the CMS draft regulations on the shared savings program for FFS Medicare recipients and its relevance to CHCs.

Improving the overall quality of health care delivery requires the creation of close partnerships with specialists and hospital partners (and others) across the care continuum working together to ensure seamless coordination of care and the timely transfer of relevant clinical information. This second session demonstrates how CHCs can create effective partnerships with their community providers to achieve optimal care coordination across the continuum of care.
