NACHC is standardizing the quality of service delivery at the health center, primary care association and health center controlled network levels through trainings, measures, and technical assistance to document real time health center performance, improvement and impact. We offer culturally tailored and integrated care management strategies, proven to improve health outcomes, streamline services and control costs. We also offer workforce training programs for new medical directors as well as in emerging clinical models, such as Patient Centered Medical Homes, and in topics such as population health management.
NACHC also invests in Quality Infrastructure for optimized workflow, health information technology, and expanded care teams. Our newly formed Quality Center is a national collaboration of stakeholders to build bridges for improvements in the patient and staff experience, costs and equity of care for health center communities.
