The 2018 Payment & Delivery Reform Summit will create a space for primary care associations (PCAs) and health-centered controlled networks (HCCNs) to learn, brainstorm, troubleshoot, and identify the key action steps needed to support health centers in value-based payment and practice transformation. Learn how health centers are coming up with new and unique ways to address the needs of their patients and the communities they serve by using patient-centered care models to address clinical and non-clinical needs alike, and the strategies being used by PCAs and HCCNs to help support and advance those efforts. Then discover how health center payment can best reflect the models of care for health centers in your state.

As U.S. Secretary of Health and Human Services Alex Azar recently said, health centers are not just partners in value-based care, they are “pioneers already.” At the 2018 Summit, glean inspiration from some of the pioneering reforms taking place at health centers with guidance and support from PCAs and HCCNs, and then dig through the nuances with national experts and tailor next steps for payment and delivery reform to your state.
