Practice Transformation and Performance Improvement Boot Camp

The team-based care model is not new to health centers and is now the foundation for all successful practice transformation. During this session, participants will learn how to staff and manage the care team in order to both construct and sustain a culture of excellence and high-performance. You will learn strategies and best practices to reduce burnout, improve the patient and staff experience, and identify Key Performance Indicators (KPI) to benchmark and track operational success. Participants will learn the role of leadership, and management, in aligning the team-based model across departments. The session will be interactive with hands-on activities to enrich the learning experience.

Learning Objectives:

  • Evaluate current workflows to reduce care team burnout.
  • Validate performance improvement through the application of Key Performance Indicators (KPIs).
  • Recommend a care team model to exceed operational and clinical goals.
  • Understand the LEAN model to improve operational efficiency.

Navigating the Medicare FQHC Cost Report โ€“ Are you ready?

The training will focus on the purpose of the Medicare FQHC cost report in the environment of the Medicare FQHC Prospective Payment System (PPS). Session participants will walk through the Medicare FQHC cost report (Form CMS-224-14) to gain an understanding of the overarching changes from the previous version of the Medicare FQHC cost report (Form CMS-222-92). This session will also focus on the expanded reporting of FQHC identification, statistical and other data via Worksheet Series โ€œSโ€. Organization information is being collected for a variety of purposes, including assisting CMS in the future development of a FQHC market basket index. Then the training will highlight the importance of reclassifying and adjusting costs on the Medicare FQHC cost report in accordance with Medicare cost reporting principles documented in the Medicare Reasonable Cost Principles (42 CFR Part 413) and the Provider Reimbursement Manual (CMS Publication 15-1) .

  • Explain the flow of worksheets in the new Medicare cost report.
  • Distinguish between the current and previous versions of the Medicare FQHC cost report.
