Overview
Heart disease and stroke remain the leading causes of death in the United States. NACHC and the CDC aim to achieve Million Hearts® goals in partnership with health centers across the country. Strategies focus on improvements in the use statins with high-risk patients, blood pressure management and control for African Americans, and the use of self-measured blood pressure (SMBP) monitoring.
