The annual Community Health Center Chartbook presents data on nation’s Health Centers, including Health Center Program Grantees and Look-Alike organizations. The chartbook includes summary data on health center patients, services, staffing, quality outcomes, and impact on access to care.
The chartbook also compares health center outcomes to other providers and national data. Unless otherwise noted, chartbook data comes from the 2023 Uniform Data System, collected by the Health Resources and Services Administration.
The chartbook contains nearly 90 pages of charts illustrating different aspects of health centers and their patients, from dental services onsite to percentage of patients with chronic conditions, and much more.
