In Memoriam and Honoring Legacies
Remembering Those Who Shaped the Community Health Center (CHC) Movement
Submission Deadline: Saturday, July 18, 2026
During CHI – Community Health Conference, NACHC will feature an In Memoriam tribute celebrating the lives, leadership, and lasting contributions of individuals who dedicated themselves to advancing community health and serving patients across the country.
If your health center, Primary Care Association (PCA), Health Center Controlled Network (HCCN), or organization has experienced the loss of a leader, board member, clinician, advocate, or other valued member of the movement within the last year, we encourage you to submit their information for consideration.
Please include:
- Full name
- Organization and title/role
- Date of passing (if available)
- A high-resolution photo
- A brief remembrance, biography, or note highlighting their contributions to the Community Health Center field (100–200 words preferred)

The National Association of Community Health Centers (NACHC) recognizes individuals and organizations whose leadership, service, innovation, and dedication have significantly advanced the mission and impact of Community Health Centers across the nation.
