Two people from any single health center, PCA or HCCN who are responsible for developing, adopting and/or sustaining an innovation. Depending on the innovation you wish to work on during the event, it might be individuals related to operations, finance, workforce, care teams, technology or other areas. 

During this day-long event, your team will learn from insightful panel discussions and then roll up their sleeves and work with their peers on impactful roundtable activities focused on stabilizing their own specific innovation by operationalizing it and sustaining it for the future

$300 per person.  

To make the most of the event we recommend that you come ready with an innovation that your health center is currently working on – something that you need help operationalizing and/or sustaining. Prior to the event, you will receive a quick survey about your innovation. This will help pair you with other attendees facing similar issues.
