The United States is facing a maternal health crisis. According to the Centers for Disease Control and Prevention, the maternal mortality rate increased for Black and Hispanic women from 2018 to 2021. Community Health Centers are well positioned to increase access to maternal care. In addition, health centers focusing on providing wholistic and culturally responsive to patients before, during, and after pregnancy. NACHC is working with several partners to improve postpartum care for health center patients by using human-centered design to redesign workflow and HIT (health information technology) systems around the peri- and postpartum periods. We are establishing a model for health centers to partner with State Perinatal Quality Collaboratives (PQCs), identify high-risk patients, reduce gaps in care, and improve adherence to evidence-based guidelines.
