Value-Based Payment: Move Beyond the Visit – Position for Total Cost of Care NACHC is pleased to announce the launch of a Value-Based Payment (VBP) learning series designed to meet health centers and health center partners, along the VBP readiness continuum – from Planning, to Implementing, to Optimizing.
This 3-part webinar series will guide health center leaders and care teams through practical steps to design and implement a Total Cost of Care (TCOC) operating model that supports community health management, value-based payment readiness, and financial sustainability.

Session 1: Building the Foundation for Total Cost of Care Date

- Define attributed populations, accountability structures, and units of analysis (PMPM/PMPY)
- Map key cost drivers across the care continuum
- Establish a baseline Total Cost of Care for attributed patients
Session 2: Using Risk and Data to Guide Care and Cost Strategies

- Stratify patient populations by clinical risk and utilization
- Align care management resources and workflows to risk tiers
- Deploy targeted interventions to reduce avoidable utilization and improve outcomes
Session 3: Modeling Financial Performance and Sustaining Results

- Apply risk adjustment across cost domains and model financial scenarios
- Use cost insights to inform value-based contract negotiations
- Monitor TCOC performance and refine interventions through ongoing
