Goal & Mission
Champion CHC analysis and studies that demonstrate ROI, improve patient access and outcomes, and increase funding.
To deliver actionable analyses with partners that strengthen care delivery for high-risk populations, demonstrate CHC value, and support data-driven policy, practice, and education.
Our Key Aims:
Measuring CHCs’ financial, clinical, and social ROI, including cost savings, utilization trends, quality outcomes, and community impact.
Expanding research collaboration to increase federal and private research funding opportunities that improve access to high quality, affordable, medical, pharmaceutical, and social interventions for populations at high risk of poor health.
Convening CHCs, PCAs, HCCNs, NTTAPs and external partners to set research priorities.
Expanding and leveraging health information technology use, data analysis, and research tools to address critical practice, educational, and policy questions.
Recognizing CHCs, HCCNs, PCAs excelling in key research areas to serve as resources for expertise and collaboration.
Obtaining financial support for CHC research infrastructure, fellowships, and other key partnerships to further expand and enhance the CHC model.
