Overview:

NACHC’s Quality Center is pleased to announce the launch of two new courses for health center professionals to support systems transformation and advancement toward the Quintuple Aim. These courses have limited availability with no registration fee. 

Course Information: 

  • Lifestyle Coaching: Intended for health center staff with some knowledge of the National Diabetes Prevention Program who are interested in facilitating a PreventT2 lifestyle change program. Learn more here.
  • Healthy Weight and Diabetes Prevention Consultation: Intended for health center staff interested in enhancing care for persons with higher weight, including those with diabetes and related conditions. Learn more here.

Eligibility: 

  • Health center staff member
  • Registered for the Elevate National Learning Forum
  • Commitment to participate for the full duration of the program
  • Commitment by health center leadership that three or more staff have/will complete the Value Transformation Framework (VTF) Assessment before the training course begins
