Overview
This webinar series is a collaborative effort between the National Association of Community Health Centers and the American College of Lifestyle Medicine, supported by funding from the Ardmore Institute of Health.
This five-part webinar series provides a comprehensive, step-by-step exploration of how Lifestyle Medicine Shared Medical Appointments (LMSMAs) can transform chronic disease care within community health centers. Designed for clinical, operational, and programmatic leaders, the series illustrates how evidence-based lifestyle interventions, scalable care models, and interprofessional teamwork can strengthen whole-person care while advancing value-based outcomes.
Beginning with a foundational introduction to Lifestyle Medicine and the LMSMA model, the series establishes a shared understanding of the principles, evidence base, patient populations, workflows, and readiness considerations essential for successful implementation. Participants then dive more deeply into four leading LMSMA aligned curricula—Full Plate Living, the Lifestyle Empowerment Approach for Diabetes Remission (LEADR), Supervised Lifestyle and Integrative Medicine (SLIM) for weight loss, and Diabetes Self Management Education and Support (DSMES)—each offering practical approaches to supporting behavior change, enhancing patient engagement, and empowering clinical teams across diverse care settings.
The AAFP has reviewed Integrating Lifestyle Medicine into Health Center Transformation, and deemed it acceptable for AAFP credit. Term of approval is from 05/19/2026 to 06/23/2026. Physicians should claim only the credit commensurate with the extent of their participation in the activity.
AAFP Prescribed credit is accepted by the American Medical Association as equivalent to AMA PRA Category 1 credit(s)™ toward the AMA Physician's Recognition Award. When applying for the AMA PRA, Prescribed credit earned must be reported as Prescribed, not as Category 1.

Webinar 4: SLIM (Supervised Lifestyle and Integrative Medicine) Weight Loss

This webinar session four of five provides an overview of the Supervised Lifestyle and Integrative Medicine (SLIM) model and its application within group visits for patients with obesity and other chronic conditions. Participants will learn how to integrate the SLIM curriculum with medical management—including the use of GLP-1 receptor agonists—to support sustainable behavior change and improved outcomes. The session will also outline practical strategies for implementing SLIM in clinical practice, including care team roles, patient engagement approaches, and key operational and billing considerations to support scalable, team-based obesity care.

Webinar 5: Diabetes Self-management Education & Support (DSMES)
June 23, 2026
This final webinar session of the series introduces the key components of the Diabetes Self-management Education & Support (DSMES) framework and its evidence-based approach to supporting chronic disease self-management. Participants will learn how to apply DSMES core elements to support chronic disease self-management through Lifestyle Medicine Shared Medical Appointments (LMSMAs) across diverse clinical settings. The session will also explore strategies for integrating DSMES principles into LMSMAs to support patient self-management, including sustainability planning, stakeholder engagement, and defining measurable outcomes.
