Bethesda, MD—The National Association of Community Health Centers (NACHC) and Health Evolution have announced a new partnership to prioritize Community Health Center (CHC) patients and convene CHC leaders to participate in national forums focused on cutting-edge delivery solutions that drive health system change and advance health for all.
This announcement comes as Health Evolution Connect 2025 kicks off in Nashville, with 16 CHC leaders in attendance. The NACHC and Health Evolution collaboration will ensure CHC leaders are “in the room” and engaged in cross-industry discussions held in-person and virtually at year-round Health Evolution marquee events, including the Summit and Forum.
CHCs are the foundation of health care in our nation’s medically underserved communities delivering affordable, effective, comprehensive primary care that is community-driven and improves health for all.
