Heart to Heart: Empowering Patients in their Blood Pressure Control (Webinar, English)
Self-Measured Blood Pressure Monitoring (SMBP) Implementation Toolkit (Toolkit, English)
The Controlling Hypertension Learning Series is a learning collaborative focused on using the Practical, Robust, Implementation and Sustainability Model (PRISM) which targets: (1) guideline-informed treatment intensification; (2) improving follow-up intervals; (3) improving medication adherence; and (4) increasing uptake of self-measured blood pressure monitoring (SMBP). Experts from the American Medical Association, NACHC, and health centers share learnings and guidance on these strategies to improve blood pressure control. Here, we offer access to the webinars and slide presentations offered during that learning collaborative.
