The Program of All-Inclusive Care for the Elderly (PACE) is a fully capitated Medicare–Medicaid
model that delivers comprehensive medical, behavioral health, long-term services and supports (LTSS), and social services to nursing-home-eligible older adults. The program is designed to allow participants to remain safely in their community while receiving coordinated, team-based care.
The PACE program aligns closely with Community Health Centers’ (CHCs’) mission
to provide comprehensive, community-based care to underserved populations,
particularly older adults with complex needs. CHCs serve a large and growing Medicare
population, including nearly 3.6 million Medicare beneficiaries and over one million dually
eligible individuals.1 As the nation’s largest primary care system, CHCs are essential partners in integrated care models like PACE, given their role in serving older, medically complex patients in underserved communities.
